The demo starts with Grace tied upside down on a hospital bed. She cuts herself free using broken glass pieces and escapes the medical facility. Players explore an abandoned hospital filled with locked doors and electrical puzzles. Grace carries only eight items at once and must find keys to progress through different areas.

Combat works differently from other Resident Evil games because Grace lacks military training. She throws bottles at enemies instead of using guns or knives. A large monster hunts Grace through the hospital and can bite chunks from her body. Players can heal damage using medical injectors found throughout the building.
