The Auditor-General's Report reveals that radiotherapy machines scheduled for September 2024 arrival remain undelivered as of January 2025. Twenty cancer patients experienced postponed treatments lasting eight weeks due to technical problems. The linear accelerator system cannot integrate with the hospital's existing ARIA technology platform. Equipment incompatibility prevented installation and testing procedures from proceeding.

Evaluation committees rejected the supplier's proposal during procurement processes. Technical assessments identified integration failures before contract approval occurred. The tender procurement committee overruled expert recommendations and selected 0074062-D despite system compatibility issues. Alternative bidders offered higher technical scores for minimal additional costs.
