Pakistan's Tafenoquine cure fails as Global Fund cuts aid

Pakistan successfully concluded its pilot program for Tafenoquine, a single-dose antimalarial medication that represents a major advancement toward the country's malaria elimination target by 2035. The Ministry of Health conducted the trial to establish safe administration protocols for this newly approved treatment option. Health officials presented findings from the G6PD screening initiative at a formal event organized by the Directorate of Malaria Control in Islamabad. The program demonstrated Pakistan's leadership role in regional malaria eradication efforts. State Minister for Health Dr. Mukhtar Ahmed Bharath described the pilot as foundational work for future scalable interventions.

The revolutionary medication treats Plasmodium vivax malaria with one dose rather than the traditional 14-day Primaquine treatment regimen. Many patients abandoned the lengthy Primaquine course after two or three days, creating risks for disease relapse and continued transmission. Tafenoquine eliminates these compliance issues by delivering complete treatment in a single administration. The Drug Regulatory Authority of Pakistan approved the medication in September 2024 following World Health Organization endorsement. This approval positioned Pakistan among the first regional countries to adopt this advanced therapeutic approach.

The pilot program operated across nine districts with high malaria burden, receiving technical assistance from Medicines for Malaria Venture. Health workers integrated Glucose-6-Phosphate Dehydrogenase testing into primary care facilities to prevent hemolytic anemia in patients with enzyme deficiency. Pakistan reported over 2.8 million malaria cases following the 2022 floods, emphasizing the urgent need for improved treatment strategies. The Global Fund reduced Pakistan's health program allocation by more than $27 million for the current grant cycle. This funding decrease reflects global donor constraints and concerns about program management effectiveness.
